To register for an examination, download this PDF registration form. Registration fees are $85 per exam. Send the registration form with your check or money order (payable to Testing Services, Inc.) to:

To avoid an additional $15 late fee, the registration must be received at least three business days prior to exam date requested.

The registration fee for walk-ins is $100. No personal checks will be accepted at the exam site.

Registration Confirmation

Confirmation of test date will be sent to the address or fax number noted on the registration form.

Refund and Rescheduling Policy

Exam fees are non-refundable.

Registrants who fail to show up for the scheduled exam are considered "no shows" and forfeit the examination fee.


If an exam must be rescheduled for any reason, it must be coordinated through Testing Services, Inc. by calling (601) 607-7736. Registrants can reschedule their examination a maximum of 2 times with the 2nd reschedule being assessed a rescheduling fee of $15.00. This fee will be collected upon your check-in on the day of the exam. You must reschedule no later than 2:00 p.m. on the business day prior to your scheduled exam. If a holiday occurs on this day, you will have to reschedule the business day prior to the holiday date.


Current photo identification (driver's license, school or work ID or passport) and your Social Security card is required. A current 1099, W-2 or tax return may be substituted for the Social Security card.
